Sports Forms for School Sports
DMS CLUB & ACTIVITY FORM
Dear Parent/Guardian:
The Spring sports season, including boy's and girl's lacrosse, softball, baseball, and some intramurals will start immediately after school starts. Any student who wishes to try out for a sport must have a physical exam on file in the nurse's office. The physical must have been completed within 365 days of the first day of tryouts for any sport. If the physical was done at the time of a previous sport season, a Seasonal Sports Update form must be signed by a parent and turned into the nurse before tryouts.
Students cannot attend tryouts unless their paperwork is complete.
Sports physicals are not offered at school. Students should have their physicals at their own “medical home” (family physician/advanced practice nurse). If obtaining a physical poses a financial hardship for you, please contact the school nurse as soon as possible to make alternative arrangements.
Please note the following important information:
- All physicals must be completed using the standard forms required by New Jersey.
NO OTHER FORMS WILL BE ACCEPTED. Forms are available in the nurse's office, and may be picked up at the middle school office during the summer (office closed on Fridays.) Forms can also be downloaded and printed from the middle school website under “Activities” at www.evesham.k12.nj.us/dms
- Physical exams may not be done by a chiropractor.
- Sports physicals must have been completed within 365 days of the first day of tryouts for any sport.
- All physicals must be reviewed by the school physician, per regulation. Therefore, they must be submitted by the deadline. Please note that any late physicals may not be processed before tryouts start, and the student will not be eligible to participate in tryouts.
- If your child might need emergency medication during after-school activities, (an inhaler or Epipen), a self-medication form must be completed by your physician. Forms are available on the middle school website under “Nurse”.
- Intramural sports also require a sports physical per state mandate.
PLEASE CHECK YOUR FORMS BEFORE LEAVING THE DOCTOR'S OFFICE TO MAKE SURE THEY ARE COMPLETE. The Physician must sign both the physical AND the cardiac clearance forms.
ALL REQUIRED FORMS MUST BE SUBMITTED TO THE SCHOOL NURSE.
